The practice is registered and complies with the Data Protection Act 2018. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.

You have a right to access the information we hold about you, and if you would like to access this information, you will need to complete a Subject Access Request (SAR). Please ask at reception for a SAR form and you will be given further information. There may be a charge to access records or have copies of notes, please contact the Practice Manager for further information.
Furthermore, should you identify any inaccuracies, you have a right to have the inaccurate data corrected.
